CURAVITA AND INSURANCE

Many extended health plans cover all or part of the cost of chiropractic care, physiotherapy, registered massage therapy and custom orthotics, including plans from Canada Life, Sun Life, Manulife and the Public Service Health Care Plan (PSHCP). Coverage and annual limits vary from plan to plan, so we recommend checking your benefits before your first visit.

We submit your extended health claim directly to your insurer, so your reimbursement arrives sooner. Payment is due at the time of your visit, and your insurer reimburses you according to your plan. For custom orthotics, most plans require a physician’s prescription; please check with your plan before your assessment.

Please contact either clinic if you have questions about your coverage.

MOTOR VEHICLE ACCIDENTS, WSIB AND OTHER PLANS
We bill WSIB and auto insurers directly for approved care, and we also work with plans for members of the RCMP, Veterans Affairs Canada and the Canadian Armed Forces. Details for each are below.

After an accident, it is important to be examined as soon as possible, as some injuries only show symptoms later. Report the accident to your auto insurer and complete your Application for Accident Benefits (OCF-1) as soon as you can.

When you book, let us know the visit is related to a motor vehicle accident so we can schedule extra time for your assessment. Please bring your claim number, your adjuster’s name and contact details, and any forms your insurer has sent you. We prepare and submit your treatment plan forms and bill your auto insurer directly for approved care.

Accident benefits from your auto insurer cover approved treatment after a motor vehicle accident. The amount available depends on your injuries and your policy, and your adjuster can confirm what applies to you.

For more information on accident benefits in Ontario, visit the Financial Services Regulatory Authority of Ontario (FSRA) website.

RCMP members may receive chiropractic care without a prescription. We bill Medavie Blue Cross directly for RCMP members. Please bring your Blue Cross benefit card to your first visit, and we will confirm the coverage remaining for the year. Massage therapy requires a referral from your medical doctor, renewed each year.

Veterans Affairs Canada covers chiropractic care, massage therapy and orthotics for eligible veterans, and we bill Medavie Blue Cross directly. Please bring your VAC health identification card to your first visit. Visit limits and referral requirements are set by VAC; massage therapy and orthotics require a referral from your physician. If more care is needed, your practitioner will request approval from VAC and submit progress reports as required.

Chiropractic care and massage therapy are available to Canadian Armed Forces members when referred by the member’s CAF medical officer. Care must be authorized before treatment begins, and it is the member’s responsibility to obtain that authorization. Please bring your referral to your first visit.

We bill WSIB directly for your approved care. Please bring your WSIB claim number to your first visit. If you do not have one yet, our front desk can help with your Form 6.
